Supply Chain Business Intelligence: The Critical Need for End-to-End Visibility in Supply Chain Management

Supply chains are the lifeblood of industries with logistical components, where even minor disruptions can ripple into significant financial losses and customer dissatisfaction. According to the BCI Supply Chain resilience Report, disruptions across industries are rising. Almost 80% of organisations’ supply chains were disrupted in the past twelve months, with most experiencing between one and ten disruptions. In today’s modern world, the complexity of interconnected systems often makes it difficult to trace transactions and identify bottlenecks. The same report states that most organisations identify the first two tiers of their supply chains as their main cause of disruption. That’s why, end-to-end visibility into these processes is critical for ensuring operational efficiency, minimising delays, and quickly resolving issues. By providing a clear view of workflows and system interactions, organisations can optimise their supply chains, enhance reliability, and maintain a competitive edge in demanding markets.

In this blog, we look at the impact of supply chain disruption on organisations and how Business Intelligence with end-to-end visibility can help overcome the complexity of modern supply chains.

In any organisation, different teams face unique challenges when it comes to managing supply chains:

Operations Teams:

Struggle with limited visibility into supply chain bottlenecks and system failures. This lack of visibility often leads to delays, increased operational costs, and the need for ad-hoc manual interventions which are not scalable and prone to errors.

Financial Teams:

Find it difficult to trace complex transactions across interconnected systems, leading to discrepancies, inefficiencies, and challenges in financial reporting. This not only affects financial reporting but also impacts cash flow and operational efficiency. According to a Splunk, companies with Observability have high-performing supply chains and can achieve 30% higher financial performance than those with less effective supply chains.

Security Teams:

Are often blind to vulnerabilities within transaction flows, making it hard to detect and address potential threats. This can lead to undetected breaches and compliance issues, posing significant risks to the organisation. The average cost of a data breach in Australia has reached a record high of AUD $4.26 million in 2024, reflecting a 27% increase since 2020, making plugging security vulnerabilities even more critical.

How Supply Chain Business Intelligence can support sectors.

At the business level, the lack of end-to-end visibility can lead to operational inefficiencies, increased costs, and customer dissatisfaction. There are key sectors where end-to-end Visibility is business critical, let’s look at these a little closer:

Finacial Services

Business intelligence (BI) is crucial for financial services as it enhances decision-making, improves customer service, and helps detect and prevent fraud. This ensures efficient operations and a competitive edge in the market.

Manufacturing

Ensuring the timely delivery of raw materials and components is critical to maintaining production schedules and meeting customer demands. Visibility helps in tracking inventory levels, managing supplier relationships, and optimising production processes.

Retail

Retailers rely on efficient supply chains to keep shelves stocked and fulfill online orders. Visibility into the supply chain helps in ensuring products to market, managing inventory, reducing stockouts, and improving customer satisfaction.

Energy and Utilities

This sector requires the efficient management of resources and infrastructure. Visibility helps in monitoring the supply chain for equipment and materials, ensuring the reliability and efficiency of operations.

Logistics and Transportation:

Companies in this sector need to manage the movement of goods efficiently. Visibility helps in optimising routes, reducing delays, and improving overall operational efficiency.

Healthcare

The healthcare sector requires precise tracking of medical supplies, pharmaceuticals, and equipment. Visibility ensures that these critical items are available when needed, reducing the risk of shortages and ensuring patient safety.

.Digitising the supply chain: The role and Benefits of Business Intelligence

Business Intelligence (BI) plays a pivotal role in modern supply chain management, offering a robust set of functionalities that empower organisations to harness data for informed decision-making. These functionalities include:

Data Collection and Integration: BI tools gather data from multiple sources such as inventory systems, transportation management systems, and customer relationship management systems. This data is then integrated and transformed into a unified format for analysis.

Data Analysis and Visualisation: BI tools utilise advanced analytics techniques to analyse the integrated data and generate meaningful visualisations, such as dashboards and scorecards. These visualisations provide a comprehensive view of key performance indicators (KPIs) and help stakeholders identify trends and patterns.

Real-time Monitoring and Alerts: BI tools enable real-time monitoring of supply chain operations by continuously collecting and analysing data. This allows organisations to identify and address issues promptly, such as inventory shortages or delivery delays. Alerts can be set up to notify stakeholders of critical events or deviations from predefined benchmarks.

Predictive Analytics: BI tools leverage predictive analytics models to forecast future demand, optimise inventory levels, and anticipate supply chain disruptions. By analysing historical data and external factors, organisations can make proactive decisions to mitigate risks and optimise resource allocation.

Collaboration and Communication: BI tools facilitate collaboration and communication among supply chain stakeholders by providing a centralised platform for data sharing, reporting, and decision-making. This improves visibility and transparency across the supply chain, enabling better coordination and faster response to changing market conditions.

Business Intelligence (BI) tools like Splunk Cloud provide a holistic view of supply chain operations and transactional workflows. They enable organisations to:

Identify Bottlenecks: Quickly pinpoint and address failures or delays in the supply chain.

Improve Operational Efficiency: Streamline processes and enhance system reliability.

Enhance Security: Monitor transactions in real-time to detect and mitigate vulnerabilities.

Reduce Costs: Not only from the resulting downtime but also from the ability to make smarter and faster decisions by analysing information across the supply chain.

Improve Decision-Making: Enable better decision-making through comprehensive data analysis.

Enhance Customer Satisfaction: Improve customer satisfaction by ensuring timely and efficient operations.

Connect Stakeholders: Facilitate connections between internal and external ecosystems.

Final Thoughts

The complexity of modern supply chains necessitates the need for end-to-end visibility. By leveraging business intelligence tools like Splunk Cloud, organisations can overcome sector-specific challenges, improve operational efficiency, and ensure the smooth functioning of their supply chains.

Our client is the advisor of choice for their global and domestic client base. It is their utmost priority to maintain their reputation, client trust, confidentiality of intellectual property, and to ensure the continuity of their exceptional services.

Our client’s organisation has a range of commercial property services, including:

Valuation and strategic advisory.

Retail, residential, hospitality and retirement real estate sales.

Commercial real estate leasing and management.

Project management.

Knowing their cyber security and privacy risk profile across all asset classes and clients plays a significant role in supporting their strategy and their ongoing compliance for regulated clients.

This requires identification and understanding of their key risks across diverse asset classes so they can prioritise the monitoring of key risks and their largest reputational and financial threats. This builds the foundation for an actionable roadmap for remediation activities.

Our client operates in a highly complex and regulated environment. Knowing their cyber security and privacy risk profile across all asset classes and clients plays a significant role in supporting their strategy and ongoing compliance. Avocado has helped them understand and remediate their greatest financial and reputational risks.

The Approach

In light of the intricate organisational landscape spanning various business services, Avocado undertook a thorough threat and risk assessment. The objective was to define a robust approach and attain a deeper comprehension of the organisation’s threat and risk profile, and potential consequences. This facilitated a prioritised approach that took into account cyber controls with the highest impact on the organisation’s key asset classes. It ensured that risk exposure was evaluated in relation to the probable cyber threat vectors confronting the organization, and a comprehensive strategy was devised to address this exposure.

Significantly, our cyber consultants:

Drawing upon our extensive domain expertise and specialised comprehension of their current cyber security controls, we maximised our ability to provide valuable insights and guidance.

Utilised the FAIR model – an internationally recognised industry standard model. This allowed us to calculate tangible financial losses that could occur if potential cyber threats against the company were to occur, enabling a more robust discussion with the board in their understanding and impact of their information management risks.

Avocado identified the crucial controls that exhibited the highest efficacy in mitigating potential losses while aligning with the organisation’s risk appetite.

Avocado successfully developed a risk-buydown matrix to showcase effective risk remediation strategies. This included:

Identifying potential cyber risk scenarios that could pose threats to the organsation’s asset classes, originating from both internal and external threat actors.

Utilising Threat, Risk, and Control (TRC) modelling to evaluate the inherent risk exposure associated with each scenario – identifying those with medium, high, or extremely severe risk severity impacts.

Illuminating the existing risk exposure for each scenario based on the organisation’s cyber security control environment.

Avocado provided comprehensive assistance to the client by efficiently targeting remediation activities for risk buy-down. Our tailored support emphasised the identification of key threats to specific business asset classes and the determination of critical controls that necessitated mitigation. This was achieved through a strategy and roadmap that seamlessly integrated with the existing IT strategy, with the desired uplift in cyber maturity.

Up Next: The Outcome >[image_with_animation image_url=”23129″ image_size=”full” animation_type=”entrance” animation=”None” animation_movement_type=”transform_y” hover_animation=”none” alignment=”” border_radius=”none” box_shadow=”none” image_loading=”default” max_width=”100%” max_width_mobile=”default”]

The Outcome

Through our meticulous threat and risk assessment (TRA), we delivered a focused and objective approach to cybersecurity planning. This assessment aligned with the organisation’s unique risks, control environment, and risk appetite.

By providing our client with an actionable roadmap that demonstrated financial justification, we facilitated the harmonisation of their IT and cyber roadmaps, enabling a more efficient allocation of resources.

By translating our clients’ risks into financial terms, we empowered them to make informed business decisions. This fostered improved communication among stakeholders and leadership, allowing the Board to fully grasp how risk decisions directly impact the organisation’s future.

As a result, our client now possesses a deeper understanding of their threat and control landscape. Avocado has played a pivotal role in minimising potential damage to client trust, preserving the confidentiality of intellectual property, and ensuring the uninterrupted delivery of exceptional services.

Presently, we continue to address their cybersecurity gaps effectively through a comprehensive strategy that encompasses managing the process of ISO certification, providing ongoing security assessments, and conducting third-party due diligence.

A major government agency’s journey with Splunk ITSI

In today’s fast-moving digital environments, government agencies are under increasing pressure to deliver seamless, reliable services to citizens and internal stakeholders alike.

One of Australia’s largest state government agencies partnered with Avocado Consulting to meet exactly this challenge: implementing a modern, scalable service monitoring and incident response framework built on Splunk IT Service Intelligence (ITSI).

One of Australia’s largest state government agencies partnered with Avocado Consulting to meet exactly this challenge: implementing a modern, scalable service monitoring and incident response framework built on Splunk IT Service Intelligence (ITSI).

What Is Splunk ITSI?

Splunk ITSI is a premium analytics and observability platform designed to provide end-to-end visibility into service health, predict performance degradation, and drive intelligent automation in IT operations. It consolidates infrastructure, application, and business data into a centralised view, allowing IT teams to act faster and more confidently.

Key features include:

Glass Table dashboards to visualise KPIs, service dependencies, and infrastructure layers in real time

Service health scoring and alerting, based on thresholds defined by operational or business impact

Notable event aggregation and correlation to reduce alert fatigue and isolate root causes faster

Integration with incident management platforms, such as Splunk On-Call, to streamline response workflows

When implemented correctly, Splunk ITSI becomes the operational command centre- or ‘central nervous system’ of an organisation’s digital ecosystem. These features make Splunk ITSI a compelling solution for organisations looking to modernise their operations, enhance service reliability, and drive proactive IT management.

According to Splunk, ITSI customers have reported:

Up to 90% reduction in Mean Time to Resolution (MTTR)

95% reduction in alert noise

45% reduction in total incidents by proactively identifying service degradation before it escalates.

”This engagement elevated the operational maturity of a major government department, laying the foundation for scaling Splunk ITSI to additional business services.”

The Challenge: disconnected monitoring, limited insights

Before engaging Avocado, the agency relied on a fragmented set of dashboards and monitoring tools. These systems provided limited visibility and often failed to convey service health in a meaningful or actionable way.

The agency’s existing monitoring infrastructure was characterised by:

Siloed tools: Multiple departments operated disparate monitoring systems, leading to inconsistent data and delayed incident detection.

Limited visibility: Legacy dashboards lacked the ability to provide real-time, business-contextual insights into service health.

Inefficient incident response: The absence of standardised alerting mechanisms hindered prompt resolution of critical issues.

These challenges underscored the necessity for a cohesive solution that could offer centralised visibility, proactive monitoring, and streamlined incident management.

As the agency’s digital footprint expanded across multiple departments and business units, operational complexity increased. Each team had its own practices and systems, which made it difficult to detect service issues early or respond in a coordinated way. The organisation required:

A centralised, real-time view of system and service performance

Role-specific dashboards to engage executives, analysts, and frontline responders

A scalable, flexible platform to grow with expanding service monitoring needs

The Solution: An End-to-End Observability Platform

Avocado Consulting was engaged to design and implement a long-term solution. The engagement, which spanned several years and departments, involved:

Deploying Splunk Core and ITSI across a wide array of systems and services

Building custom Glass Tables showing key infrastructure, application, and business metrics

Defining KPIs and service health thresholds aligned to real operational goals

Integrating Splunk On-Call for automated, intelligent incident alerting including after-hours response

Developing monthly service availability dashboards, used by Service Delivery Managers to identify trends and inform executive reporting

Providing ongoing managed services support, including continuous enhancements and platform stewardship

This approach transformed the agency’s monitoring strategy from a fragmented reactive model to a cohesive, business-aligned observability practice.

Results That Matter

Following deployment, the agency reported measurable improvements across multiple areas:

1,195+ issues proactively detected and managed, reducing business disruption

Real-time service visualisations helped reduce incident detection and response time significantly

Standardised response processes improved collaboration across departments using Splunk On-Call automation

Improved stakeholder engagement, including executive visibility into the link between IT performance and service delivery

Historical trend analysis allowed operations teams to identify and address performance degradation before it caused outages

Most importantly, the platform enabled the agency to manage growing digital complexity with confidence and clarity – laying a solid foundation for future service scaling and cross-agency collaboration.

Why it worked: Avocado’s delivery model

This engagement was more than just a software implementation; it was a strategic shift. Avocado Consulting’s methodology emphasised:

Alignment of IT metrics with business value

Stakeholder-driven delivery, involving application SMEs, project managers, architects, and operations teams

Scalable platform design, ensuring it could grow with evolving agency needs

As one of the largest Splunk ITSI deployments globally, this project is a strong example of what’s possible when technical expertise, stakeholder engagement, and modern observability practices come together.

Conclusion

Government agencies often face challenges around scale, silos, and mission-critical service delivery. This agency’s success shows what’s possible when those challenges are addressed head-on – with a unified observability solution and an experienced delivery partner. Splunk ITSI, implemented by Avocado Consulting, helped shift from fragmented tools to unified visibility, from reactive response to proactive assurance, and from raw data to actionable insights.

Through the strategic implementation of Splunk ITSI, Avocado Consulting helped the agency transition from fragmented tools to unified visibility, from reactive firefighting to proactive service assurance, and from siloed data to actionable insights.\

W

Stable environments and reliable test data are foundational to effective Quality Engineering. When they are well managed, teams can validate functionality accurately, detect defects early, and move through delivery cycles with confidence. Feedback is trusted. Results are repeatable. Releases are predictable.
W
hen they are not, everything slows down.

Unstable environments create flakiness. Tests fail intermittently. Results cannot be reproduced consistently. Teams spend time diagnosing whether a failure is a real defect or an environmental issue. Confidence in the signal declines.

At that point, delivery is no longer constrained by code quality. It is constrained by infrastructure reliability.

What Reliable Environments Enable

A stable environment provides controlled configuration, predictable behaviour, and clear ownership. Changes are managed deliberately. Dependencies are known. Integration points behave consistently across cycles.

This stability allows teams to focus on validating functionality rather than troubleshooting the platform itself. It accelerates feedback loops and reduces noise in defect triage. Most importantly, it strengthens trust in test outcomes.

High-quality test data plays an equally critical role. Accurate, representative data ensures that validation reflects real-world behaviour. Edge cases can be exercised intentionally. Coverage gaps become visible. Results are meaningful.

Together, stable environments and reliable data form the foundation of credible testing.

Without them, even well-designed automation and a strong test strategy struggle to deliver value.

The Cost of Environment Instability

When environments are poorly managed, instability becomes normalised. Uncontrolled changes occur without visibility. Configuration ownership is unclear. Infrastructure drift accumulates. Version control gets lost.

The impact is subtle but significant.

Tests begin to fail for reasons unrelated to code. Teams rerun suites repeatedly to confirm results. Defect triage meetings focus on “Is it real?” rather than “How do we fix it?”

Over time, this erodes trust. Stakeholders question whether failures represent genuine risk. Teams delay escalation because they assume instability is environmental rather than functional.

Environment instability does more than waste time. It masks actual defects. Issues that should have been identified early are obscured by noise and discovered later — often in integration, staging, or production environments where remediation is more expensive.

What appears to be a testing inefficiency becomes a delivery risk.

The Hidden Risk in Poor Test Data

Test data is frequently treated as an afterthought. Manual data creation becomes the default. Teams reuse outdated datasets. No clear synthetic or provisioned data strategy exists.

This introduces two risks.

First, poor data invalidates results. If scenarios are tested against incomplete or unrealistic datasets, coverage gaps remain hidden. Critical behaviours are not exercised. False confidence develops.

Second, privacy exposure increases when organisations rely on production data without adequate masking or governance. Sensitive information may be replicated across lower environments without appropriate controls. Compliance risk escalates alongside operational risk.

Data quality is not simply about availability.

It is about representativeness, governance, and control.

Without these, testing results become unreliable — even when execution appears thorough.

Weak Practices That Undermine Confidence

Environment and data challenges rarely stem from a single decision. They emerge from patterns of weak governance.

Uncontrolled environment changes introduce instability that accumulates over time. Without clear configuration ownership, accountability diffuses and issues persist longer than necessary.

Manual data creation limits scalability and consistency. Recreating scenarios becomes slow and error-prone. Teams cannot reliably reproduce complex defect conditions.

The absence of a defined data strategy means there is no alignment on how data is generated, refreshed, or retired. Synthetic data capabilities remain underdeveloped. Masking processes are inconsistent.

Individually, these behaviours seem manageable. Collectively, they create environments where test results cannot be fully trusted.

And when results are not trusted, decision-making degrades.

When Noise Replaces Signal

Testing is designed to surface risk early. Stable environments and representative data make risk visible.

When instability and poor data dominate, noise overwhelms signal. Teams focus on infrastructure troubleshooting rather than quality analysis. Genuine defects blend into background flakiness. Escaped defects increase not because testing is weak, but because its foundation is unstable.

The cost of late discovery rises. Remediation effort expands. Delivery schedules absorb unexpected delays.

All while the underlying issue remains environmental discipline.

What Mature Environment and Data Management Looks Like

Mature organisations treat environments and data as strategic assets, not operational afterthoughts.

Environment configurations are version-controlled and governed. Changes are transparent. Ownership is clear. Stability is measured and monitored deliberately.

Test data strategies are defined. Synthetic or provisioned data is generated to represent realistic scenarios without exposing sensitive information. Masking and privacy controls are embedded in process, not applied reactively.

Data refresh cycles are predictable. Scenario coverage is intentional. Reproducibility is built into the system.

Most importantly, environment reliability and data quality are recognised as prerequisites for credible testing. They are not delegated to the background.

The Foundation of Trust

Quality Engineering depends on trusted feedback. Automation, strategy, metrics, and skilled practitioners cannot compensate for unstable environments or unreliable data.

If your teams are rerunning tests to confirm results, debating whether failures are environmental, or discovering defects late despite extensive coverage, the issue may not be testing capability.

It may be the foundation beneath it.

Reliable environments and governed test data do more than support testing.

They protect delivery.

Because without stable infrastructure and credible data, validation becomes guesswork.

And delivery becomes exposed.

Artificial Intelligence is rapidly entering Quality Engineering. Used well, it can reduce testing time, improve defect detection, strengthen decision-making, and accelerate feedback loops across delivery. It has the potential to analyse patterns faster than humans, identify risk earlier, and optimise regression suites intelligently.
The opportunity is significant.

But for many organisations, the results fall short of the promise.

AI tools are introduced, experiments begin, and expectations rise. Yet outputs feel inconsistent. Insights are questioned. Teams struggle to trust recommendations. In some cases, workload increases instead of decreases.

The issue is rarely the capability of the technology. It is the organisation’s readiness to use it effectively.

What AI in Quality Engineering Can Deliver

When implemented deliberately, AI strengthens quality practices rather than replacing them. It can identify patterns in defect trends, predict high-risk areas, optimise test selection, and detect anomalies earlier in the lifecycle. It can reduce repetitive effort and support faster, more informed release decisions. And it can help prioritise coverage to support more effective testing.

AI enhances scale. It supports speed without sacrificing coverage. It improves signal detection within large datasets that would otherwise require significant manual effort.

But these outcomes depend on governance, data quality, and capability alignment.

AI amplifies what already exists. If processes are weak, data is inconsistent, or ownership is unclear, AI will not correct those issues. It will expose them.

When AI Adoption Increases Complexity

Without readiness, AI tools become underused, misconfigured, or poorly integrated. Outputs lack context. Models are trained on incomplete or low-quality data. Recommendations are generated but not trusted.

Teams may experiment with AI features in isolation, without a broader strategy. One group uses AI for test generation, another for defect prediction, another for automation optimisation — each operating independently.

The result is fragmentation rather than acceleration.

Instead of reducing effort, AI creates additional validation work. Teams double-check outputs manually because they do not trust the results. Productivity gains disappear.

In some cases, poorly configured models introduce bias or inconsistent recommendations across teams. Confidence declines quickly when insights appear unreliable or opaque.

Trust, once lost, is difficult to rebuild.

Indicators of Low AI Readiness

Low readiness rarely presents itself as failure. It appears as misalignment.

There is no clear AI governance model defining acceptable use, risk controls, or oversight responsibilities. Data quality standards are undefined or inconsistently applied. Teams receive minimal training and are expected to “learn as they go.”

Ad-hoc experimentation replaces strategic direction. Tools are adopted because they are available, not because they align to defined quality outcomes. Success is measured by feature usage rather than measurable impact on delivery.

These behaviours create unpredictability. AI outputs vary across teams. Models are retrained inconsistently. Compliance risks increase when sensitive data is used without clear governance or controls.

What begins as innovation becomes operational noise.

Data Quality Determines AI Value

AI performance is inseparable from data quality. Inconsistent defect categorisation, incomplete test histories, or poorly structured datasets reduce model reliability. AI cannot generate meaningful insight from fragmented or unreliable inputs.

If data governance is weak, AI will amplify inconsistency rather than eliminate it. Outputs may appear sophisticated but lack accuracy. Teams will detect discrepancies quickly, and trust will deteriorate.

Strong AI readiness requires disciplined data management. Quality standards must be defined. Ownership must be clear. Transparency must exist around how models are trained, evaluated, and refined.

Without this foundation, AI becomes experimental rather than dependable.

AI Is a Capability, Not a Feature

One of the most common mistakes is treating AI as a feature to enable rather than a capability to manage. A tool is procured, functionality is activated, and expectations are set.

But AI requires structured integration. Governance frameworks must define responsible usage. Compliance controls must be embedded. Ethical considerations must be addressed. Usage guiderails must support growth.

Teams must be trained not only in how to use AI tools, but how to interpret and validate outputs.

Organisations that approach AI strategically define clear objectives: reduce cycle time, improve defect prediction accuracy, optimise test coverage, or strengthen release readiness decisions. Measurement is aligned to outcomes, not novelty.

AI adoption without governance increases risk. AI adoption with governance increases scalability.

What Strong AI Readiness Looks Like

Mature organisations treat AI as an enterprise capability within Quality Engineering. Governance structures are established. Data standards are enforced. Training equips teams to use AI responsibly and effectively.

AI use cases are prioritised based on measurable business value. Pilots are evaluated rigorously. Outputs are validated before scaling. Transparency is built into processes so teams understand how insights are generated.

Safe and compliant usage is not an afterthought. Privacy, bias mitigation, and security controls are integrated from the outset.

Most importantly, AI initiatives are aligned to delivery objectives. The question is not whether AI is being used, but whether it is improving speed, predictability, and risk visibility.

AI Should Strengthen Judgment, Not Replace It

AI has the potential to transform Quality Engineering. It can reduce manual effort, accelerate defect detection, and support smarter decisions.

But it does not replace discipline.

Without governance, data integrity, and capability alignment, AI introduces variability instead of clarity. It increases workload instead of reducing it. It erodes trust rather than strengthening it.

If your AI initiatives feel experimental rather than enabling, the issue may not be the tool itself.

It may be readiness.

Because AI does not create maturity. It amplifies it.

And when readiness is strong, AI becomes a powerful accelerator of quality and delivery. When it is weak, it becomes another source of uncertainty.

The difference is not technology.

It is preparation.

Cyber security frameworks play an important role in strengthening organisational resilience.

They provide structure, shared language, and recognised benchmarks for uplift.

Cyber security frameworks play an important role in strengthening organisational resilience.

They provide structure, shared language, and recognised benchmarks for uplift. In Australia, frameworks such as the Essential Eight, ISO standards and sector-specific requirements are widely adopted – and rightly so.

However, frameworks are most effective when they are applied at the right point in the journey. Before an organisation can meaningfully align to any framework, it needs a clear, realistic understanding of its current cyber maturity and risk context. That is where a cyber security maturity baseline becomes critical.

A baseline is not a framework – and it isn’t meant to be

A cyber security baseline does not replace frameworks. Its purpose is different.

A baseline establishes:

Where the organisation is today

How cyber maturity operates in practice

Where risk is genuinely concentrated

Which areas require attention first

Frameworks, on the other hand, define:

What “good” looks like

Which controls are expected

How uplift can be structured, assured and measured

When frameworks are applied without a baseline, organisations often encounter:

Overly ambitious uplift plans

Investment in controls that do not address the highest risks

Fatigue from trying to do too much, too quickly

Difficulty sustaining maturity over time

A baseline provides the context frameworks need to be applied proportionately and effectively.

When organisations typically look for a cyber security baseline

Organisations usually seek a cyber security baseline when one or more of the following triggers are present:

Preparing for upcoming governance, resilience or regulatory expectations, including CPS 230

A recent cyber incident, near miss or control failure

Increasing reliance on third parties, cloud platforms or SaaS providers

Board or executive questions such as “Are we actually resilient?”

Limited budgets combined with higher accountability for outcomes.

In these moments, the challenge is rarely a lack of frameworks. It is a lack of clarity on where risk truly sits.

Why timing matters more than the framework itself

Frameworks are designed to be broadly applicable. Organisations are not. Every environment has its own mix of:

Technology complexity

Operational dependencies

Regulatory pressure

Third-party exposure

Delivery velocity

Risk tolerance

Without understanding these factors, framework adoption can become a compliance exercise rather than a risk-reduction strategy.

A baseline answers a different question:

“Given how we actually operate today, where does risk truly sit?”

Only once that is understood does it make sense to determine:

Which frameworks are most relevant

Which controls should be prioritised

What level of maturity is realistic in the near term

Many cyber security maturity assessments fall into one of three traps.

1. Checklist-led scoring

They measure maturity against a single framework- often Essential Eight or ISO- and stop there. This shows whether controls exist, but not whether they are effective in context.

2. One size fits all targets

Organisations are pushed toward the same maturity end state, regardless of size and complexity, sector and regulatory exposure and operating model or delivery velocity.

3. Theoretical uplift

The outcome is often a score and a long list of controls, with little clarity on what reduces risk first, what can wait, or what adds cost without improving resilience. This creates a false sense of security: “We’ve done Essential Eight, so we’re fine,” or “We’ll never be ISO-ready, so what’s the point?” Both increase risk, just in different ways.

A domain-based baseline reflects real-world risk

Modern cyber risk rarely sits in a single control gap.

It emerges across multiple, interconnected areas – technical, operational, governance-related and organisational.

A domain-based baseline assessment helps ensure risk is viewed holistically rather than through a single lens.

While domains vary by organisation, they typically span areas such as:

Foundational security controls

Operational and delivery practices

Governance, policy and accountability

Data and information risk

Third-party and supply-chain exposure

Emerging and evolving threats

Domains are not frameworks. They ensure the baseline reflects how risk actually manifests in real operating environments.

Essential Eight remains the foundation – not the destination

The Essential Eight continues to provide a strong foundation for cyber hygiene.

Used well, it supports:

Reduction of common attack vectors

Consistency across organisations

A clear path for technical control uplift

However, its value is maximised when organisations understand:

Their operational realities

Their exposure beyond purely technical controls

Their capacity to implement and sustain change

A baseline assessment enables Essential Eight uplift to be:

Targeted to the areas of highest risk

Aligned to organisational capability

Sequenced sensibly over time

This avoids the common trap of attempting to uplift everything at once – and succeeding at very little.

From baseline to framework-led uplift

The outcome of a baseline assessment is not a score for reporting purposes.

It is a starting position.

From that starting point, organisations can:

Select the frameworks most relevant to their context

Prioritise controls based on risk and impact

Build a practical, defensible uplift roadmap

Track progress in a way that makes sense to technical and non-technical stakeholders

This sequencing – baseline first, frameworks next – supports sustainable improvement rather than reactive change.

Reducing cyber risk without unnecessary complexity

Cyber resilience isn’t about finding every possible gap.
It’s about fixing the ones that actually reduce risk.

A baseline assessment helps organisations understand:

Where they stand today

What matters most in their operating context

Which actions deliver real risk reduction

All without over-engineering controls or adding complexity that looks good on paper but fails operationally. Cyber security maturity is not achieved by implementing the most controls.

It is achieved by implementing the right controls, at the right time, for the right reasons.

Establishing a baseline provides the clarity needed to make informed decisions and ensures that frameworks are used as enablers, not obstacles.Understanding where you are today is what allows frameworks to work as intended.

It is not about lowering standards. It is about applying them intelligently.

Translate technical controls into clear, business-ready language your executives understand

Align cyber initiatives with governance, risk and compliance expectations

Prioritise actions that deliver real risk reduction and measurable value

Move from reactive firefighting to confident, data-driven decision-making

Right-size recommendations to your industry, threat exposure, organisational scale and risk appetite- no unnecessary controls

Start with clarity. Then uplift with confidence.

Automation is intended to accelerate delivery. It promises faster feedback, improved consistency, and reduced manual effort – allowing teams to focus on higher-value work, rather than repetitive regression cycles. When implemented well, automation strengthens confidence and increases predictability.

However, many organisations discover that their automation estate is not delivering those outcomes. Execution times grow. Maintenance effort increases. Defect discovery rates begin to drop. Releases slow down rather than speed up. Confidence in automated results begins to erode.

This is rarely a failure of technology. It is usually a failure of strategy, governance, and lifecycle thinking.

What Effective Automation Is Meant to Do

Effective automation supports delivery flow and shortens feedback loops. It validates critical paths consistently across environments, and reduces reliance on large manual test phases late in the cycle. Over time, it lowers cost by shifting effort from execution to prevention.

Most importantly, automation done well improves decision-making. When automation is stable and well-designed, teams can rely on rapid, trustworthy feedback to assess readiness. That reliability is what enables faster, safer releases.

Automation’s value is not in the volume of scripts created, but in the confidence it provides.

When Automation Becomes the Constraint

Poorly implemented automation produces the opposite effect. Test suites become brittle, with small application changes triggering widespread failures. Maintenance consumes increasing capacity. Teams spend more time fixing automation than analysing risk. Existing tests become flaky, and degrade, demanding more and more effort to keep them running.

Execution pipelines slow under the weight of expanding regression packs. In some cases, releases take longer than they did before automation was introduced. What was meant to streamline delivery becomes a bottleneck within it.

This typically occurs when automation grows organically without architectural discipline. Coverage increases, but coherence does not. The estate expands, but strategy does not keep pace.

The Impact of Weak Governance

A lack of standardisation or tool governance compounds these challenges. Teams select tools independently, often based on personal familiarity rather than organisational need. Frameworks evolve differently across programs. Duplicated tooling and conflicting approaches become common.

Over time, knowledge fragments and cross-team collaboration weakens. Onboarding slows because each team operates differently. Shared improvement becomes difficult because there is no shared foundation. Integration between automated suites becomes difficult, and scalability gets lost.

Tool diversity is not inherently problematic. Unmanaged diversity is. Without alignment, friction increases and velocity declines.

Common Behaviours That Undermine Automation

Automation issues often stem from patterns that seem practical in isolation but problematic in combination.

Without a clear automation roadmap, investment becomes reactive. Teams automate what is immediately visible rather than what is strategically important. High-risk areas may remain under-automated, while low-impact functionality receives disproportionate attention. Defects escape to production due to inconsistently applied coverage.

Automating unsuitable test cases further destabilises the suite. Highly volatile UI interactions or edge cases that change frequently introduce fragility and inflate maintenance effort. When automation is heavily concentrated at the user interface layer, feedback loops become slower and more sensitive to change than necessary.

Frameworks that are not maintained degrade gradually. Dependencies become outdated. Execution environments drift. Stability declines incrementally rather than dramatically, making the problem harder to detect early.

Tool selection driven primarily by developer preference can also create long-term inconsistency. While individual productivity may improve temporarily, systemic alignment suffers.

Individually, these behaviours appear manageable. Collectively, they erode the effectiveness of automation as a delivery enabler.

Automation Is a Product, Not a Project

One of the most significant misconceptions is treating automation as a one-off initiative. A framework is implemented, coverage targets are met, and the effort is considered complete.

In reality, automation requires ongoing lifecycle management. It demands ownership, governance, periodic refactoring, and incremental improvement. As systems evolve, automation must evolve with them.

When automation is not actively maintained as a living capability, debt accumulates. Maintenance effort increases. Trust declines. Teams begin to bypass failing tests rather than address underlying issues.

At that point, automation no longer accelerates delivery. It complicates it.

What Mature Automation Looks Like

Mature automation is aligned to risk and delivery outcomes. It is guided by a roadmap that prioritises business-critical paths. The automation capability is treated as an investment in future quality levels. Coverage is balanced across layers, reducing over-reliance on UI testing and strengthening lower-level validation.

Frameworks are standardised where appropriate, and tool selection is governed deliberately. The automation estate is reviewed and refined over time, not simply expanded.

Most importantly, success is measured by impact on delivery. Has cycle time improved? Has defect escape reduced? Has release predictability increased? Has manual effort decreased sustainably?

If those outcomes are not improving, automation may be active but not effective.

Automation Should Enable Flow

Automation is one of the most powerful levers within Quality Engineering. When designed and governed intentionally, it increases delivery speed, consistency, and confidence. It allows teams to focus on higher-value work and strengthens the organisation’s ability to scale.

When implemented without strategic alignment, it becomes brittle, fragmented, and expensive to maintain.

If your automation capability feels heavy rather than enabling, the issue is unlikely to be the tools themselves. It is more likely a lack of roadmap, governance, and lifecycle thinking.

Because automation is not about writing scripts.

It is about engineering confidence at scale.

And when it fails to do that, it is not accelerating delivery — it is constraining it.

Why your test cycle keeps blowing out (and it’s costing every release) - Avocado Consulting - deliver with certainty

Most teams don’t struggle because they can’t write tests. They struggle because they can’t run them cleanly, quickly, and consistently.

When execution maturity is low, it shows up in the same ways every time: longer cycles, late surprises, and release decisions made on opinion instead of evidence. If quality is uneven across teams, execution is often the clearest divider. One team can validate change quickly. Another is stuck in slow, manual, noisy runs that don’t build confidence.

This post focuses on Test Execution – a foundational capability that underpins consistent quality at scale. Regardless of how good your test design is, or your planning, execution determines whether testing supports fast, confident delivery – or slows teams down with noise, rework, and doubt.

Fast feedback is the real output of test execution

Efficient, well-planned execution exists for one reason: rapid feedback the team can act on while the work is still fresh.

Mature execution means your testing cadence matches your delivery cadence. You can run checks early and often. Failures are understandable. Fixes happen before the sprint turns into a fire drill.

Your execution model should help you answer one question quickly:

Are we still safe after this change?

Industry benchmarking reinforces that execution speed and reliability are not nice-to-haves — they are core quality signals. The Sauce Labs Continuous Testing Benchmark (2024) highlights short, predictable test run times as a prerequisite for fast feedback.

You don’t need to adopt specific benchmark numbers to apply this insight. When test execution is slow or unstable, feedback degrades and everything downstream slows with it: triage takes longer, fixes arrive later, reruns multiply, and release decisions are made with less confidence. Test execution maturity determines whether testing accelerates delivery or quietly becomes the bottleneck. [image_with_animation image_url=”29072″ image_size=”full” animation_type=”entrance” animation=”None” animation_movement_type=”transform_y” hover_animation=”none” alignment=”” border_radius=”none” box_shadow=”none” image_loading=”default” max_width=”100%” max_width_mobile=”default” img_link=”https://avocado.com.au/tools/devsecops-maturity-self-assessment/”][image_with_animation image_url=”29069″ image_size=”full” animation_type=”entrance” animation=”None” animation_movement_type=”transform_y” hover_animation=”none” alignment=”” border_radius=”none” box_shadow=”none” image_loading=”default” max_width=”100%” max_width_mobile=”default” img_link=”https://avocado.com.au/tools/devsecops-maturity-self-assessment/”]

Poor execution turns testing into a bottleneck and a confidence problem

Poor execution practices introduce delays, create bottlenecks, and reduce team confidence in release readiness. It creates noise and friction in the delivery lifecycle.

This rarely starts dramatically. The regression pack grows. Environments are shared, but increasingly perceived as unstable. Test data is … “nearly ready”. Failures bounce between people because ownership is unclear. The team compensates with longer hardening periods and more manual checking.

Then it becomes normal.

The cost is not just time. It’s decision quality. When results arrive late, teams face binary trade-offs: slip the release, or accept risk. When results are noisy, teams waste time debating whether failures are real. Testing becomes something to “get through” rather than an identifier of risk, and a feedback loop that guides delivery.

This is one of the most common causes of uneven quality. Some teams ship with evidence. Others ship with hope.

The behaviours that quietly break execution maturity

Low execution maturity usually comes from a small set of predictable behaviours.

Unprioritised test runs

If every run is treated as equal, execution time expands until it consumes the sprint. Teams run big suites by habit, not because the change justifies it. Feedback arrives too late to help.

Mature teams prioritise runs. They run high-signal checks early and frequently, and reserve deeper runs for the moments where they add value. Execution becomes deliberate rather than exhaustive.

Delayed delivery of code

Every team has been there. The schedule is in place. The tests are written. But the developers just need “one more day” before they provide the code to the testers. One day becomes two, and soon the execution window is reduced.

When mature execution processes are in place this can be handled, but without them what began as a delay can soon become panic – and delivery suffers as a result.

Lack of test data readiness

Execution slows to a crawl when data provisioning is manual, inconsistent, or unsafe. Teams end up blocked, or they “make it work” with shortcuts that create risk later.

In our experience, test data management is a recurring bottleneck because teams need data that is representative, accessible, and safe. When data isn’t ready, even well-designed test plans stall, making data readiness the difference between predictable delivery and delayed release decisions.

Execution slows not because teams can’t run tests, but because data isn’t available when it’s needed. Teams wait on refreshes, approvals, masking, or manual workarounds, turning test execution into a stop-start activity and eroding confidence in release readiness.

Unclear ownership of failures

When nobody owns failures, execution becomes a loop of handoffs. Developers assume it’s a test issue. Testers assume it’s a product issue. Failures linger. Reruns grow. Fingers point. The pipeline becomes something the team works around. Clear ownership isn’t blame. It’s a defined path for triage, root cause, and remediation so execution stays predictable.

Excessive reliance on manual execution where automation is feasible

Manual execution has a place. Over-reliance does not. When stable regression checks remain manual, feedback slows, repeatability drops, and release readiness becomes dependent on individuals being available. Automation is not about removing judgement. It’s about making repeatable checks fast and consistent, so humans can focus on investigation, edge cases, and risk-based thinking.

This is what enables continuous testing in practice – not as a goal, but as a by-product of execution that is reliable, repeatable, and always ready to run.

The maturity gap indicators you can spot without a dashboard

High levels of test flakiness, inconsistent execution logs, and ad-hoc scheduling are strong signals of maturity gaps.

Flaky tests are especially corrosive because they destroy trust in the signal. Recent practitioner research and academic work continues to show the same outcome: flaky tests disrupt CI, increase investigation time, and reduce productivity. Teams rerun failures, ignore failures, or stop treating the pipeline as evidence.

Inconsistent execution logs create a similar problem. If failures don’t provide clear, actionable information – what changed, where it failed, what data was used, and how to reproduce – triage turns into guesswork. The cost isn’t just a slower fix. It’s slower releases, because nobody is confident.

Ad-hoc scheduling is the third tell. If regression runs happen irregularly, late, or only when someone remembers, regression challenges surface when they’re hardest to fix. Mature execution makes scheduling boring: consistent triggers, predictable cycles, and a clear release readiness rhythm.

What good execution delivers for ROI and team health

Effective execution ensures predictable cycles, reliable results, and fast detection of regressions. That has direct ROI implications because it reduces unplanned work and protects delivery capacity.

When execution is mature, teams:

catch regressions early (when changes are small and fixes are cheap)

avoid extended hardening periods

reduce reruns and triage churn

release with evidence rather than debate

can adapt and respond to delivery changes, rather than react and panic

When execution is not mature, rework grows quietly. Engineers’ context switch into late fixes. Testers become traffic controllers. Delivery and support costs rise. Governance overhead increases, because release confidence drops and leaders demand more assurance.

In Australia, there’s also a business risk layer when execution shortcuts involve customer data. The OAIC’s Notifiable Data Breaches guidance makes clear that breaches involving personal information can trigger notification obligations when serious harm is likely. If test execution depends on unsafe data practices, the cost isn’t only delay. It can become an incident with regulatory and reputational consequences.

The message for leaders is straightforward: slow, noisy execution is not just a QA issue. It’s a productivity issue, a predictability issue, and sometimes a governance issue.

What to do next

Test Execution is one of nine maturity domains Avocado uses to assess your overall Quality Engineering Maturity.

If your test cycle keeps blowing out, don’t assume you need more people or another tool. Measure maturity first. Our Quality Maturity Assessment benchmarks capability across all nine domains and highlights where execution is creating bottlenecks – run prioritisation, data readiness, failure ownership, flakiness, logging quality, and scheduling discipline.

Why testers and developers don’t trust each other - Avocado Consulting - deliver with certainty

Quality rarely fails because people don’t care. It fails when ownership is fragmented – when quality is treated as everyone’s job, but no one’s responsibility.

Quality is a team sport, not an end-phase.

Quality rarely fails because people don’t care. It fails when ownership is fragmented – when quality is treated as everyone’s job, but no one’s responsibility.

Quality is a team sport, not an end-phase.

In high-performing quality organisations, trust between testers and developers isn’t left to chance. It’s built into how teams work across business, development, testing, and operations. This is anchored by a shared quality strategy that defines what “good” looks like across the organisation, not team by team.

The defining trait of uneven quality environments is this: effort exists everywhere, accountability exists nowhere.

Well defined cross-functional collaboration can prevent issues early, before the release train is already moving. The business clarifies what “good” looks like in customer terms. Developers build in smaller slices and validate behaviour as they go. Testers shape scenarios during refinement, so coverage is aimed at the risky paths, not guesswork. Operations brings production reality into the conversation early: monitoring, alerting, rollback, and what failure looks like under real traffic and real data. Feedback loops exist to continually improve the performance of teams, and move the organisation forward. Without this shared ownership model, teams default to role-based optimisation – speed here, coverage there, stability somewhere else – and quality fractures along organisational lines.

When this alignment exists, teams aren’t guessing how to prioritise quality, they’re executing against a common intent.

This isn’t “culture work” for its own sake. It links to measurable delivery outcomes. DORA’s research has long used throughput and stability metrics such as change failure rate and time to restore service. When those stability measures improve, teams usually have fewer last-minute surprises because collaboration across roles is tighter and feedback arrives earlier.

In practice, DORA’s stability metrics improve not because teams test more, but because quality decisions are shared between business, developers, testers, and operations. These teams work as one, aligning early on risk, expectations, and ownership.

In Australia, the stakes are often higher because customers expect digital services to work first time, every time. Many industries also operate under strict operational risk expectations. For APRA-regulated entities (banking, insurance, super), CPS 230 commenced on 1 July 2025 – lifting focus on resilience and operational risk management. This reinforces the need for a clear, shared quality strategy that aligns delivery teams to business risk, not just technical completeness.

That makes “quality as a team outcome” a business requirement, not an engineering preference.

A few signals you’re operating in this mode:

Risk is discussed early in plain language the business understands.

Testers and developers agree on what “done” means through shared conversations early in delivery, creating a common understanding of risk and expectations before code merges. Those expectations are derived from an agreed quality strategy, not local convention.

Operations isn’t a downstream recipient. It’s part of release readiness.

The handoff habit that turns delivery into “throw it over the fence”

Poor collaboration creates siloed responsibilities, misaligned expectations, and the “throw it over the fence” mindset. It rarely starts with conflict. It starts with how work is structured, such as when strategy is absent or unclear – and teams are left to fill the gaps themselves. Part of this can be the result of work practices where teams have been given full autonomy to work as they please. Whilst this can be satisfying tactically, it results in mismatches and friction between teams at scale.

The “throw it over the fence” mindset is not a cultural flaw, it’s a structural outcome of unclear ownership. When responsibility for quality is split across roles without shared accountability, handoffs become the default operating model. A mindset of “once it’s out of the team, it’s someone else’s problem” develops, and overall quality suffers.

When work is organised as a handoff chain, each group can feel like they’re doing the right thing. Business writes requirements and expects delivery to match intent. Development builds to an interpretation of those requirements. QA receives work late and is expected to validate under deadline pressure. Operations is told a release is coming, rather than involved in making it safe.

Each step makes sense in isolation. Collectively, they create uneven risk coverage and inconsistent outcomes.

Then the language changes. People stop saying “we” and start saying “they”.

You’ll recognise the lines:

“Dev is done, it’s with QA now.”

“QA is blocking release.”

“It passed in test, so it’s not our issue.”

“Ops will deal with it if anything breaks.”

That’s the handoff habit. It creates friction because responsibility is split from control. It also pushes discovery late.

This is where uneven quality becomes expensive. Issues aren’t prevented, they’re discovered late, debated under pressure, and paid for repeatedly across rework, delays, and incidents.

It also exposes the absence of a shared quality strategy – there is no agreed view of where risk should be reduced, so it gets debated late instead. Late discovery is what turns normal defects into trust problems, because the team is forced to debate risk when there’s no time to respond calmly. Teams begin to react in haste – rather than respond in collaboration.

This is where a metric lens helps. DORA defines change failure rate as the proportion of deployments that require intervention after release, such as a rollback or hotfix. In “throw it over the fence” environments, this failure rate rises – not because teams lack effort, but because risk is managed late and in isolation, and quality is applied unevenly rather than strategically.

Inconsistent delivery

Some releases feel fine. Others turn into a scramble. Releases increasingly have to be backed out, or ‘deployed dark’ with unfinished features toggled off in production. Testing windows shrink. Defects pile up late. Retesting is rushed. Scope gets cut at the end. Dates move, and confidence drops.

Developers feel it as churn: rework, context switching, and last-minute merge pressure. Testers feel it as responsibility without time: you’re expected to provide confidence when the system gives you no room to create it. The business feels it as forecasting pain: commitments become hard to trust, and every release needs extra reassurance.

Knowledge gaps

Knowledge gaps form when the right people aren’t in the same conversation early enough.

Testers don’t always get the “why”, so they test broadly rather than targeting the paths that matter most to customers. Developers don’t always see operational constraints, production data patterns, or integration behaviour, so features behave well in test but fail under load or with real data.

The symptoms are predictable:

Testers are not involved in the design stage

Defects are raised without enough context to prioritise quickly.

Fixes land without shared understanding of what else might be impacted.

The same issues repeat because root causes aren’t owned end-to-end.

Adversarial relationships

Once pressure is constant and context is fragmented, defect conversations can feel personal even when no one intends that.

Developers experience late defect discovery as disruption. Testers experience late involvement as being set up to fail. Operations experiences incident load as risk being pushed downstream. Business experience late defect discovery as increased cost, and decreased confidence in the process. At that point, trust isn’t lost because people are unreasonable. Trust is lost because the system produces repeated surprises.

Most organisations don’t choose mistrust. They create it through a small set of repeatable behaviours.

When testers first see work after it hits a test environment, they become the messenger of bad news. They find issues when the cost of change is highest and the timeline is tightest. Developers feel ambushed. Testers feel stuck validating risk they didn’t help manage, and forced into the role of the gatekeepers of production.

If decisions live in private messages, scattered notes, and inconsistent tools, context fragments. Defects become debates because people aren’t aligned on what “done” means, what the acceptance criteria protect, and which risks are acceptable. Reporting and metrics are difficult to produce and lack context, as there is no clear source of truth for progress and status.

If “quality” belongs to QA, development is pushed toward output and QA is pushed into gatekeeping. That creates role conflict.

Shared ownership changes the tone. It means testers and developers agree on risk and “done” early. It also means automation is treated as a team asset, not a separate stream. You see fewer debates about whether a defect is “valid” and more focus on what it means for customer outcomes and release confidence. [nectar_icon icon_family=”fontawesome” icon_style=”default” icon_color_type=”color_scheme” icon_color=”Accent-Color” icon_padding=”0px” pointer_events=”all” icon_fontawesome=”fa fa-window-restore” icon_size=”30″ margin_bottom=”20″]Ceremonies only help if they produce shared understanding.

When testers aren’t in refinement, risk thinking arrives late. When developers aren’t part of triage, defects become “someone else’s problem”. When ops aren’t in release planning, monitoring and rollback are afterthoughts.

A small number of high-quality rituals prevent a lot of reworks: refinement that includes test scenarios and edge cases, triage focused on impact and patterns, and release planning that covers operational readiness, not just deployment steps.

High-performing teams invert this model. Quality is embedded into collaboration, not inspected at the end. QA is involved early, accountability is shared, and cross-functional ceremonies are used to surface risk before it materialises – not to assign blame after it does.

The production bill: rework, slower cycle time, and releases nobody trusts

The true cost of uneven collaboration is rarely visible immediately. It accumulates quietly through rework, elongated cycles, and incidents that feel avoidable in hindsight.

A poor quality culture increases rework, prolongs cycle time, and decreases trust in released software through increased production incidents.

The mechanism is consistent. Late discovery drives rework. Rework displaces planned work. Planned work slips. Pressure rises. Shortcuts creep in. Incidents increase. The next release starts under more pressure, with less trust.

This is the point where Culture & Collaboration becomes a ROI discussion. Not because culture is “soft”, but because the cost shows up as unplanned work, incident response, and recovery effort.

A clear quality strategy and approach makes this easier to explain to leaders because it puts language and measurement around what teams feel every week:

DORA frames stability through change failure rate and time to restore, which reflect how often releases create production pain and how quickly teams recover.

Splunk’s downtime research (with Oxford Economics) has reported very large aggregate costs for major organisations and cited high cost-per-hour figures, reinforcing that instability drains money, time, and executive attention.

CISQ’s work on the cost of poor software quality and technical debt supports the reality that quality problems compound. Over time, systems become slower to change and riskier to release.

For a delivery team, this lands in practical ways people recognise straight away:

Defect-driven rework keeps displacing roadmap delivery.

Cycle time grows through fix–retest loops and release delays.

Production incidents become normalised, and recovery slows when operational readiness is thin.

Stakeholder confidence drops, which increases governance overhead and slows decisions.

If you’re explaining uneven quality to leadership, this is the cleanest line to draw: uneven quality is often a symptom of uneven maturity in how teams collaborate and share ownership of risk.

What to do next

Culture & Collaboration is one of the most powerful drivers of quality outcomes because it shapes how risk is surfaced, shared, and acted on across delivery. When teams still rely on handoffs and late discovery, additional tools and frameworks rarely change the result.

The practical next step is to measure where quality breaks down today – across people, practices, and ways of working – so improvement effort is focused where it will reduce delivery rework and production incidents.

Take our Quality Maturity Assessment to identify where quality is uneven across teams, and receive a clear, prioritised set of actions to reduce production incidents, shorten cycle time, and restore release confidence. (and it’s costing every release)

Learn why poor execution is blowing out your test cycle and how to fix it.

Defects that keep coming back don’t just slow delivery. They change how teams behave.

Developers become cautious about touching “fragile” areas. Testers lose confidence in what “fixed” means. Product and delivery leads start padding timelines, because release quality feels unpredictable.

Defects that keep coming back don’t just slow delivery. They change how teams behave.

Developers become cautious about touching “fragile” areas. Testers lose confidence in what “fixed” means. Product and delivery leads start padding timelines, because release quality feels unpredictable.

That spiral is often misdiagnosed as a capability problem. However, in our experience, it’s usually a defect practices maturity problem: how defects are captured, triaged, prioritised, reported, learned from, and ultimately prevented from recurring.

This post examines defect management and why low-maturity practices surface quickly, sitting at the centre of feedback loops, decision-making, and release readiness.

Defect practices that prevent repeat pain

Strong defect practices help teams detect issues early, prevent recurrence, and improve product quality and user satisfaction. The support teams to focus on fixing the issue, rather than argue over who caused it.

The difference is discipline and feedback loops.

In mature teams, defects are treated as structured signals about risk. Teams don’t just “close the ticket”. They ask: what allowed this to happen, how do we stop it happening again, and what should change in coverage, monitoring, or design?

This structured approach drives fewer production defects, fewer support contacts, and fewer repeated customer frustrations. Users feel that as a service that behaves consistently, especially around the workflows that matter: onboarding, payments, account access, claims, scheduling, and changes to permissions or pricing.

In Australia, the bar for reliable digital services is explicit in government guidance. The Digital Performance Standard includes a focus on maintaining a reliable service that is “available, stable and consistent” for users. Even outside of government, customers bring the same expectation to banks, telcos, retailers, and SaaS.

When triage drags, releases become unpredictable

Poor defect management results in long triage cycles, repeated issues, and unpredictable release quality.

Long triage cycles usually mean the team is paying interest on every defect.

A bug is raised, then waits. It gets re-read, reclassified, and re-explained. People try to reproduce it with different data and different environments. It bounces between “needs more info”, “can’t reproduce”, “not my problem” and “works as designed”. Each bounce adds time, frustration, and risk.

While triage drags:

fixes land later, when they are harder to test safely

defects stack up and compete for attention

teams ship with “known issues” because schedules don’t move

release confidence drops because evidence is thin

The same root causes then create repeat defects. The organisation ends up with two backlogs: the bug backlog you can see, and the risk backlog you can’t.

The behaviours that keep defects alive

Common contributory behaviours include incomplete defect descriptions, poorly defined requirements, broken or missing triage processes, inconsistent severity/priority assessments, lack of root-cause analysis, and ignoring production incidents caused by defect leakage.

These behaviours look small in isolation. Together, they create a system where defects recur.

Incomplete defect descriptions

When reports lack clear steps, expected vs actual behaviour, environment context, and supporting evidence (logs, IDs, screenshots), triage becomes guesswork. Guesswork increases cycle time and encourages shallow fixes.

Poorly defined requirements

Ambiguity turns defects into debates. Teams spend time arguing intent rather than resolving a clear mismatch. This delays fixes and encourages inconsistent decisions across teams.

Broken or missing triage

If triage is ad hoc, defects age and become harder to resolve. Mature teams make triage routine: set cadence, clear roles, fast first response, and a predictable path to “fix, defer with justification, or close with evidence”.

Inconsistent severity and priority

If everything is urgent, nothing is. If urgent issues are treated as normal, defect leakage increases. Consistent severity/priority is what makes planning credible.

No root-cause analysis for repeats and high-impact issues

Recurrence is often a symptom of treating defects as isolated events. Root-cause reviews should be routine for repeat defects, production-impacting issues, and anything that keeps resurfacing across releases.

Ignoring production incidents

Production incidents caused by defect leakage are learning opportunities. If they aren’t fed back into triage rules, test coverage, and engineering standards, the same gaps persist.

“Accepted defects” and the debt you can’t see… yet

A culture that tolerates “accepted defects” without understanding their impact leads to degraded user trust and increased long-term technical debt.

Teams will always accept some defects. That isn’t the issue.

The issue is acceptance without clarity.

When “accepted defects” become a habit, they turn into hidden debt:

workarounds pile up in code and processes, impacting operations costs

complexity grows, making future change riskier

defects reappear because the underlying weakness remains

users repeatedly hit the same friction points, release after release

User trust erodes quietly. People stop reporting issues because they don’t expect a fix. Support teams learn scripts for “known problems”. Product teams avoid areas that feel brittle. That is technical debt with a customer-facing price tag.

The end result can mean your customers start to slip away, because there was just one too many workarounds in your product that they had to deal with.

Acceptance can be mature when it is explicit: impact, risk owner, mitigation, and review date. Without that, acceptance becomes neglect.

Late defects cost orders of magnitude more

Defects become more expensive to fix by orders of magnitude the later they are detected, making disciplined defect practices crucial.

This is a well-established lifecycle effect. NASA’s “Error Cost Escalation” paper notes that finding and fixing a software problem after delivery can be upwards of 100 times more expensive than finding it during requirements and design. The reason is simple: more work has been built on top of the defect, so the blast radius is larger. More people must be involved. More regression risk must be managed.

Late defects also create a team cost multiplier:

context switching and interruption of planned work

longer retest cycles and more coordination across roles

rushed fixes that can create new defects

more governance effort because confidence drops

Early detection is not just cheaper. It is calmer.

And the simplest version of the ROI argument is still true:

Every defect fixed prior to deployment is one your customers never saw.

That’s one less broken journey. One less support interaction. One less reputational hit. One less incident waiting to happen.

What to do next

If defects keep coming back, don’t default to “we need better people”. It may be time to measure your maturity in defect practices.

Our Quality Maturity Assessment benchmarks maturity across nine domains, including defects. It can show where recurrence is being created: defect capture quality, triage cadence, severity/priority consistency, root-cause discipline, and whether production incidents are being turned into prevention.
